Who is permitted to inject fillers and biostimulants in Norway, Sweden, Denmark and Finland? The rules are surprisingly different from country to country. Here you get an overview of competence requirements, registration and supervisory authorities in all four markets.

Section 4a of the Health Personnel Act states that only healthcare personnel may carry out cosmetic procedures, injections or treatment that can cause the patient serious injury, illness, complications or side effects. Physiotherapists may inject in certain cases, but this requires a specific assessment of competence on a case-by-case basis.
Since 1 July 2021 a dedicated act applies (lag om estetiska kirurgiska ingrepp och estetiska injektionsbehandlingar): only a legitimerad läkare, tandläkare or sjuksköterska may perform aesthetic injection treatments. The act simultaneously introduced a required reflection period and mandatory patient insurance.
Cosmetic treatment — including lip fillers — may only be performed by doctors or their medhjælp (typically nurses) with active registration with Styrelsen for Patientsikkerhed. Denmark is unique in the Nordics in requiring separate registration of both the treatment site and the individual practitioner before the business can start and be marketed.
Finland stands out clearly: unlike botulinum toxin, which requires a prescription from a doctor, there is currently no dedicated legal regulation of who may perform filler treatments. The supervisory authority Valvira has stated that it is considering guidelines, but for now it is in practice possible for practitioners without healthcare authorisation to offer such treatment — even though reputable clinics consistently use authorised healthcare personnel.

Clinics with practitioners or branches in more than one Nordic country should note that the requirements are not mutually transferable — authorisation in one country does not automatically confer the right to practise in another. Denmark has the most bureaucratic barrier to entry, with separate registrations for both clinic and practitioner, while Sweden has the strictest professional restriction with statutory penalties for breaches.
Finland currently represents an exception in the Nordics, but industry organisations and Valvira have signalled that stricter regulation may be introduced. Clinics establishing themselves in Finland now should therefore expect the requirements to converge with those of the other Nordic countries over time.
In Norway, cosmetic injections fall under section 4a of the Health Personnel Act (helsepersonelloven § 4a): only healthcare personnel may carry out cosmetic procedures, injections or treatment that can cause the patient serious injury, illness, complications or side effects. This excludes practitioners without healthcare authorisation. Physiotherapists may in some cases inject following a specific assessment of competence, but practice varies. There is an 18-year age limit for the use of fillers without medical justification.
Since 1 July 2021 the Swedish act on aesthetic surgical procedures and aesthetic injection treatments regulates this strictly: only a legitimerad läkare, tandläkare or sjuksköterska may perform aesthetic injection treatments such as fillers or botulinum toxin. There is an 18-year age limit, a required reflection period and mandatory patient insurance. Breaches of the act can result in fines or imprisonment for up to 6 months.
In Denmark, cosmetic treatment including fillers may only be performed by doctors or their medhjælp (for example nurses) with valid registration with Styrelsen for Patientsikkerhed. Both the treatment site and the individual practitioner must be registered separately, and the clinic must report all treatments to Landspatientregisteret (LPR).
Finland differs from the three other Nordic countries: as of today there is no specific legal regulation of who may perform filler treatments (unlike botulinum toxin, which requires a prescription from a doctor). Valvira is considering introducing guidelines, but for now practitioners without healthcare authorisation can in practice offer filler treatment in Finland, even though reputable clinics use authorised healthcare personnel regardless.
Yes in Denmark, where both the treatment site and the practitioner must be registered with Styrelsen for Patientsikkerhed before the business can start. In Norway and Sweden it is primarily the individual practitioner's authorisation that applies, while the business is regulated by general requirements for sound healthcare services. In Finland there is currently no equivalent registration obligation specifically for filler treatment.
Sweden and Denmark introduced their own laws and registration schemes for cosmetic treatments after a period of growing concern about patient safety in a previously lightly regulated industry. Norway regulates it through the general Health Personnel Act. Finland has not yet adopted equivalent specific legislation for filler treatments, and the supervisory authority Valvira has stated that it is considering whether guidelines should be introduced.
